NEW PATIENTS

Your first visit to the practice of John Otrhalek, DDS, FAGD establishes a vital and lasting foundation for our relationship with you. During that visit we will go over your medical history and important background information and provide you with sufficient time to get to know Dr. Otrhalek and his wonderful team. For your convenience, we have provided a welcome packet with additional information about our office and to save you time at your first visit, you can print out and complete the patient forms in advance. Please complete those forms and bring them with you to your appointment along with any records and x-rays from your previous dentist. Doing so will help expedite your dental care. We will schedule you for a convenient appointment with our restorative, cosmetic and family dentist in Harper Woods. At that appointment we will discuss your needs, be they restorative or cosmetic treatment, teeth whitening in our office or your home, or simply education of proper dental hygiene and care.
